Post Acute Care

One of the medical care services which help in a patient's continued recovery from disease or the treatment of a chronic illness or disability are referred to as post-acute care

Transition Care Management

Services for Transitional Care Management (TCM) deals with the transition of an inpatient into an outpatient setting. Following an admission to the hospital or another type of inpatient facility (i.e. skilled nursing facility), the patient will be adjusting to a new diagnosis, a change in their medication treatment, or another health concern and it is mostly managed by primary care physicians.
